Named GP

Did you know that every patient has a named GP?

This is the GP who is responsible for handling all the paperwork for that patient (prescriptions, clinic letters etc.)

For vulnerable patients who would benefit from continuity of care, this is the GP with whom they would book routine appointments.

To find out who your named GP is, please ask reception.

Are you a new patient with us?

If you wish to register as a new patient please print off the Registration Form and New Patient Questionnaire and bring the completed forms to the surgery together with two forms of identification which include your current address. After this is complete, a nurse will then carry out a current health check.

Would you recommend our Practice to your friends and family?

We would like to know more about what you think about the practice. We have a “Friends and Family” survey running from December 2014 onwards which we would be very grateful if you would spare some time to complete. We will publish results of the survey regularly on our website. The “Friends and Family” test is also available on paper in the practice.
